Your First Visit...

At Rice Family Chiropractic we take every step possible to provide you with safe and effective Chiropractic care.

The first visit consists of a consult, spinal scan, orthopedic and neurological examination. If indicated from the examination, X-Rays may be taken.

Starting with the first visit and continuing through your care, we will also provide education on how to maintain a healthy body.

We will need you to fill out a brief medical history as well. If you like, you can download and print this form and fill it out before you arrive.
